Health Insurance Opt-Out Waiver Instructions
Domestic and International J1 students who are enrolling in 9 or more credits for Fall or Spring Semester and have compliant health insurance can elect to opt-out of SHIP by submitting a waiver each Fall and Spring semester by the posted deadline. To opt out of SHIP each semester, your insurance coverage will need to be in effect by February 1 for Spring and September 1 for Fall, and must meet all University waiver requirements in order to be removed from SHIP coverage.
The official means of communications is the MSU Denver email system. Health Insurance policy reminders are sent from [email protected] to the student’s official school email account, please add this email address to your safe senders list to be sure important reminders are read.
SPRING 2026 – HEALTH INSURANCE WAIVER INFORMATION
The Spring semester waiver will be available after October 27 for registered students taking 9 credits or more. Please wait 24 to 36 hours for your registration eligibility to be uploaded to have waiver link access in the Student Hub.
The Spring opt-out waiver deadline is February 5 , 2026. After the deadline, students registered for 9 credits or more, without an approved waiver on file, will be enrolled in SHIP for the semester.
Opt-out Waiver Instructions:
- Visit MSU Denver Student Hub at msudenver.edu/studenthub
- Click Web Services
- Under Web Services, select Student Profile (Self-Service Banner)
- Once on your Student Profile page, go to Banner Menu (4 squares at top left)
- Click Menu > Students > Health Insurance Waiver
- Click the link “Submit a Waiver or View a Waiver” to be directed to the waiver form hosted by Elev834. An additional log-in authentication may be required before being directed to the Elev834 waiver form.
- Fill in the required insurance information fields using your health insurance card.
- An Authorized Representative is optional. Select Yes to add someone you know, allowing them permission to inquire about your waiver audit, otherwise bypass this option by selecting No.
- Sign (type) your name in the signature box at the bottom of the form. Important! Type (sign) your name to match exactly how it’s showing at the top of the waiver form under Personal Information. Include any middle name, initial, period, or hyphen when signing the form, otherwise the form will show a red error message and not submit. The Personal Information fields are locked and will not accept any demographic changes. If necessary, access your Student Hub to make an address change and the University will update your waiver address automatically. Please notify the Registrar’s office if you have a name or date of birth update because it may require documentation.
- Final step is to submit your waiver by clicking the Submit button below the Signature box. A confirmation will appear if no errors were found after clicking the Submit box. In addition, you will receive a confirmation email in your MSU Denver email box, confirming you have successfully submitted the waiver and it’s pending for audit.
Please allow 10-15 business days for a waiver audit to be completed. A verification email is sent to your MSU Denver email account stating the status of your waiver audit – it will show either approved, denied, or a re-submit of information. Approved waivers will have the premium charge credited on the student’s tuition and fees account within 72hrs after the approval is issued.
Please read all ELEV834 email notices to be informed of any action that may be necessary to help complete the waiver audit. Once a waiver is approved, the student health insurance charge is removed from the student’s tuition and fees account.
