The Health Center at Auraria is committed to the privacy and confidentiality of our patients.


Privacy Practices
 

All patients have a right to keep their health information private. The Health Center at Auraria is required by law to maintain your protected health information (PHI) abiding by the most current Notice of Privacy Practices. It’s our goal to maintain the highest level of confidentiality and privacy for all of our patients. Every disclosure of your PHI requires your consent, unless they are for reasons outlined on our Notice of Privacy Practices, for your treatment, for payment, for healthcare operations or information mailed directly to your address on file.

A Notice of Privacy Practices is provided to all patients when completing the registration paperwork, but we are happy to schedule time to discuss your rights with you if you have additional questions.


HIPAA and Confidentiality
 

The Health Center at Auraria provides care to patients in a private and confidential setting. HIPAA (Health Insurance Portability and Accountability Act of 1996)  is a federal law that protects private health information of patients and is enforced by the Health Center at Auraria. All records are confidential and cannot be released to anyone without a patient’s written authorization. The only exception is when records are legally subpoenaed or required by public health law.

Patients are able to release their Health Records through the Medical Records Department by completing the Authorization for the Use or Disclosure of Protected Health Information.


Outside Referrals
 
If needed, the Health Center at Auraria providers make referrals to outside providers when additional services are needed.
